BEHAVIOR/MANNERS FORM. Please complete the provided Behavior/Manners Form. Be as detailed as possible. This will allow me to assess your dog, your relationship with your dog, the environments in which your dog lives and what behaviors/manners should be addressed. By doing so, I will create a custom program for you and your companion that will provide you both with the tools needed to live in synchronicity.
CLINICAL NUTRITION FORM. Please complete the provided Clinical Nutrition Questionnaire. Be as detailed as possible. This will allow me to assess your dog and formulate what dietary/nutrition concerns that should be addressed. By doing so, I will create a custom program for you and your companion that will provide you both with the tools needed to live in health and harmony.
Nutritional Protocol custom tailored to your dog’s individual needs
The initial Nutritional Consult fee is $150 – flat rate. After we speak, I will create a customized Nutritional Protocol specifically designed for your dog. This detailed canine nutritional analysis will be tailor made and designed to achieve optimum health for your dog’s individual needs. After the Nutritional Protocol is completed, we will schedule a follow-up phone consult to discuss and review your Nutritional Protocol. This will be billed at the standard $120 per hour telephone consult fee. You will not be billed for this part of the service until we finish our phone call so I can generate an invoice based on our specific call length.

Telephone and Video Consultations are also available for Behavior, Obedience and Nutrition issues. One 60-minute consultation is $120. After purchasing, please fill out the appropriate form (below).
