This letter of agreement is between Tamara Tokash and client. Tamara agrees to offer a customized nutritional analysis for a flat rate of $200.00 and a follow up phone consult to review findings and present a specific nutritional protocol at a rate of $100/hour. Incidental questions/ remarks will be addressed via email. If there are additional questions, comment or concerns regarding the nutritional protocol that warrant a phone consult, the charge is $120/hour.
